Request for Use of Church Bus

INSTRUCTIONS:

The Organization is required to do the following:

  • 1. All bus requests must be 14 days in advance.
  • 2. Replace fuel used.
  • 3. Make sleeping arrangements for the bus driver when the trip requires staying overnight.
  • 4. Leave the bus clean upon return.
